Caraphernelia is a broken-heart disease where somebody leaves you but leaves all of their things behind. I don't think it has anything to do with a girl named Cara mixed with another word.

He refers to the person he loves as a dizzy dreamer, and to her new love interest as her bleeding little blue boy because he is still scarred from his loss. I side with an earlier commenter on the meaning of "I brought a gun as the preacher tired to stop me," and I think this talks of him coming to the wedding brandishing a weapon and telling his love that he cannot deal with her being with another.

She wants him to leave her alone, and keeps telling him to forget her, but he can't because all of her stuff is around his house. Only at this point, as in Bulletproof Love, his affections have reached obsession. "I'll burn your name into my throat" refers to his commitment with her, yet he wanted to make her hurt now that she's left him ("I'll be the fire that will catch you"). He knows that he should "pick up the pieces" and move on with his life, but he can't seem to do so because he relied on her to be there. Now, none of the things that used to make him happy can anymore.

The next bit of the song is him realizing that he is a dispicable person, or perhaps repeating it to himself.

During the second chorus, we get a "Give her back to me. You know I can't afford the medicine that feeds what I need." This could be a reference to him needing therapy/help or perhaps needing somebody else to love.

He thinks she is throwing him away, although he is so devoted, and finally he wonders... is his life even worth picking up the pieces of?
