Apr 15th 2020!⃝I think the song represents disorders that are built upon social phobia etc, and fear of judgement, abandonment and embarrassment. And things like these are so overwhelming that they end up struggling with voices either from things like schizophrenia (or anything including psychosis- voices only existing in the victims head) or thoughts that are self hate etc. And either one or both of these bring them under the surface separating sadness (underneath) and happiness (above).
